Position Summary

The primary purpose of the Care Case Manager is to provide integrated coordination and case management functions within a multidisciplinary service delivery model for students experiencing disruption in their normal course of study due to the presence of behavioral, physical, mental health, or psychiatric-related issues; crisis situations; or incidents of bias and unwanted forms of harassment— all of which directly impact the quality of students’ ability in the learning environment.

The Care Case Manger contributes to the creation of a healthy, positive campus environment by implementing and administering the case coordination efforts and systems that support students through challenges and enhance student care and well-being, including collecting, tracking, and evaluating confidential student information pertaining to behavior, well-being and student success; communicating with campus partners; administering policies and procedures; and monitoring and performing necessary follow up actions and interventions for cases and relevant stakeholders.

This position serves as a primary point of contact for the coordination of case management functions to the Care and Advocacy Program, the Student Care Team (SCT), and the Basic Needs Program. While not a clinical position, the Care Case Manager plays a key role in coordinating and administering case management systems, facilitating the flow of information to various campus stakeholders, ensuring the integrity of confidential records, and providing administrative oversight for follow-up actions for cases and situations in the DOS case tracking database system (Maxient) and in Titanium, an electronic health record system used by confidential advocates. The Care Case Manager is a vital member of the Office of the Dean Students and must exhibit collegial and appropriate engagement in a team environment, and possess the ability and commitment to work with students, families faculty and campus partners from diverse backgrounds.

Minimum Requirements

• Bachelor’s degree.
• One year of professional experience working in an educational setting (two years graduate-level experience in an educational setting can substitute for one year of professional experience).
• One year of experience using database system software.

Preferred Qualifications

• Master’s degree in higher education administration, student affairs, counseling, social sciences, or related field.
• Work experience in higher education setting.
• Work experience in case management involving student conduct, student care, or crisis intervention.
• Professional experience in managing database systems for case management or related purposes.
• Demonstrated knowledge of student case management standards and processes in student care team coordination.
• Demonstrated experience working in a fast-paced setting that handles urgent situations concerning student well-being.
• Experience with developing, delivering, and assessing outreach and educational programs on a wide range of topics.
• Experience analyzing data and reporting trends in writing and through data visualization.
• Proficiency in Maxient (case management database software), including the ability to teach and train campus partners in navigating case management system and relevant software.
